Pharmacokinetic–Pharmacodynamic Modeling of Severity Levels of Extrapyramidal Side Effects With Markov Elements

چکیده
A major problem in the treatment of schizophrenic patients with current antipsychotic drugs, mainly acting as dopamine-2 receptor antagonists, is the occurrence of side effects such as extrapyramidal symptoms (EPS). Meta-analyses of summary data of EPS occurrence, and receptor occupancies inferred from mean plasma concentrations, have shown the incidence of EPS to rise when receptor occupancy is above ~80%. In this analysis, individual longitudinal EPS data from 2,630 patients participating in one of seven different trials and treated with haloperidol, paliperidone, ziprasidone, olanzapine, JNJ-37822681, or placebo were analyzed using a continuous time probability model with Markov elements. The developed pharmacokinetic-pharmacodynamic model describes the longitudinal changes of spontaneously reported EPS-related adverse events and their severity levels rated by clinicians. Individual steady-state concentrations and occupancy levels were found to be predictors for EPS. The results confirm 80% occupancy as a level of increased EPS occurrence rates, also at the individual level.CPT: Pharmacometrics & Systems Pharmacology (2012) 1, e1; doi:10.1038/psp.2012.9; advance online publication 26 September 2012.

Pharmacokinetic-pharmacodynamic Modelling of Side Effects of Nitrendipine
The modelling aim in this study was to explore the relationship between plasma concentrations of a cardiovascular drug nitrendipine and occurrences of its side effects. The data were taken from a bioequivalence study in 40 men with a 20 mg single dose of nitrendipine. During the study occurrences of side effects, their severity, onset time and duration were recorded. Overall, side effects occur...
متن کاملPharmacokinetic/Pharmacodynamic (PK/PD) Relationship-based Approach to Understand the Variability of Central Neurological Effects of Ethanol
Pharmacokinetic/pharmacodynamic (PK/PD) relationships describe the quantitative relationships between the drug-induced effects and the concurrent corresponding drug concentrations in an individual. Since a long time, rough qualitative relationships have been described between neurological presentation and plasma ethanol concentration in acute ethanol ingestion. However, to date, precise quantit...
متن کاملPharmacokinetics and Pharmacokinetic/Pharmacodynamic Modeling of Filgotinib (GLPG0634), a Selective JAK1 Inhibitor, in Support of Phase IIB Dose Selection
BACKGROUND AND OBJECTIVES Filgotinib (GLPG0634) is a selective inhibitor of Janus kinase 1 (JAK1) currently in development for the treatment of rheumatoid arthritis and Crohn's disease. While less selective JAK inhibitors have shown long-term efficacy in treating inflammatory conditions, this was accompanied by dose-limiting side effects. Here, we describe the pharmacokinetics of filgotinib and...
متن کاملTrace elements status in psoriasis and their relationship with the severity of the disease
Background: Psoriasis is a common and chronic inflammatory skin disease that has profound adverse effects on patients’ wellbeing. Trace elements are essential to biochemical processes in the body and are involved in immunological and inflammatory reactions such as keratinization and melanin formation. The aim of this study was to evaluate the essential metals in psoriasis in comparision with he...
متن کاملElderly's Medical Therapy Status
Studies show that the elderly are more prone to chronic diseases in comparison to other age groups. Medical treatment is very common in aged people. On average, every aged person uses 4.5 prescribed and 2.1 over the counter medicines. And every year they have 12 to 17 prescriptions. Indeed, medicines are mostly used in hospitals and geriatrics. (1) the most common used medicines are: analgesics...
